Munich is an amazing city to visit and since you are living here, you want your loved ones in the family to come and visit you and the city. In this guide, we will talk about how to fill up the application form and statement for declaration of commitment.
We have written a guide “A guide to obtain Verpflichtungserklärung (declaration of commitment)” on how to take appointment and get the declaration of commitment. In this blog, we will discuss about the 2 forms.

However, we suggest downloading it from the Munich City Website as the form might be outdated.
Once you have downloaded the form, you can see that this form is in English and German.
Form 1: Let’s start with the Application form (Form for issuing a declaration of commitment)
Page 1:
Start with providing your mobile number and email address on the top left corner. Then in the second section, select the field which says “visit up to 90 days” as your visitors are allowed only to stay for 90 days max in 180 days window.
After this, you need to provide the planned entry date of your visitor(s). This is just a planned date for them to issue you the letter.
In the next table, you need to provide YOUR details – your last name, first name, date of birth and complete address in Munich. Moreover, you need to provide the information of your profession and name of the company where you are working in Munich.
Page 2:
Now you need to provide the information of the primary guest – who is directly related to you (e.g., mother, father, cousin etc.). Provide the details of the last name, first name, date of birth, place of birth, address in their home country, their nationality, passport number and your relationship to the visitor. This relationship field is a dropdown, and you need to select one option.
In the next table, you need to provide the information of the spouse of your visitor. Provide their Last name, first name, gender, and date of birth.
In the next table, fill in the information of kids travelling with your primary guest. Provide their last name, first name, gender, and date of birth. You can use additional fields if more than 1 kids are travelling.
Note: Please note that this form is only for a family, e.g., your sister, brother-in-law, and their kids (your niece and nephews). If you want to invite two separate people e.g., your 2 sisters or 2 brothers, you need to fill 2 separate forms.
Towards the end of the page, you need to enter the date and sign the form.
Form 2: Let’s fill the second form (Statement to submit a declaration of commitment)
Page 1 – Nothing to fill here. You can move to second page.
Page 2 – Just provide your first name, last name, date and sign the form.
